County Health Rankings & Roadmaps (CHR&R), a program of the University of Wisconsin Population Health Institute, focuses on analyzing and improving community health and health equity through data, policy recommendations, evidence-based solutions, and community engagement. It provides health snapshots, reports, tools, and resources to support community-led efforts to address social determinants of health and promote healthier, more equitable communities.
